Novo Nordisk has now replaced that process with a single pre-filled pen, the Wegovy HD®, which delivers the full 7.2mg in one weekly injection. Your 4-week supply is now four pens, one per week, each pre-loaded and ready to use.

How to use the Wegovy HD® 7.2mg pen
The Wegovy HD® is designed around simplicity. There’s no needle to attach, no dial to set, and no plunger to press. The injection is triggered by pressing the device flat against your skin. Here’s the full process:
Before each injection
- Check the label and expiry. Confirm it reads Wegovy 7.2mg and is within its use-by date.
- Inspect the solution. Look through the viewing window. It should be clear and colourless. Do not use the pen if it appears cloudy, discoloured or contains particles.
- Remove from the fridge in advance. The pen can be kept at room temperature (below 30°C) for up to 28 days once removed from refrigeration. Allowing it to reach room temperature before injecting can help reduce discomfort.
Selecting your injection site
Inject into your abdomen, front of the thigh, or upper arm. Rotating between slightly different spots within the same area each week helps prevent skin irritation and ensures consistent absorption.
Step-by-step: administering your dose
- Remove the pen cap. The needle is fully enclosed, there is nothing to attach and it will not be visible at any point.
- Press the pen flat against your skin and apply firm, steady pressure. This activates the injection mechanism. Watch for the yellow bar to begin moving.
- Hold still until the yellow bar stops moving and you hear a second click. This takes 5 to 10 seconds and confirms your complete dose has been delivered. Do not lift the pen early.
- Remove the pen. The needle cover locks automatically. You will not be exposed to the needle at any point during or after the injection.
- Dispose of the pen safely in a sharps bin. Each pen is single-use only.
Important: removing the pen before the yellow bar has fully stopped moving may mean you receive an incomplete dose. Always wait for the second click before lifting the pen from your skin.
Why a 7.2mg dose was developed
For most patients, the Wegovy dose journey begins at 0.25mg and gradually increases over several months, stepping through 0.5mg, 1mg, 1.7mg and reaching the standard 2.4mg maintenance dose.
The 2.4mg dose works well for many people. But for some patients, weight loss slows or stops at this level despite continuing treatment and maintaining lifestyle changes. The 7.2mg dose was developed to provide a clinical option for this group, rather than leaving patients without a next step.
A few things worth knowing before considering 7.2mg:
- It is not automatically the next step after 2.4mg. Your prescribing pharmacist will assess whether it’s appropriate based on your progress and response to treatment

What the clinical evidence shows
The 7.2mg dose was evaluated in the STEP UP trial, a randomised controlled study that followed over 1,400 participants with obesity for 72 weeks. Patients enrolled in the trial had not achieved their weight loss goals on the standard 2.4mg dose — making the study population directly relevant to patients considering a dose increase.
Key findings from the STEP UP trial:
- Mean body weight reduction of approximately 20.7% at 72 weeks
- Over one-third of participants lost 25% or more of their starting body weight
- Weight loss continued progressively across the full study period, without an early plateau
To contextualise these numbers: the standard 2.4mg dose achieves around 15% average weight loss across the major clinical trials. The 7.2mg dose offers a meaningful additional reduction for patients who need it, approximately 5 to 6 percentage points more on average, while the semaglutide safety profile remains consistent with the established evidence base.
What to do if you miss a dose
Missing a single injection is unlikely to significantly affect your overall progress, but how you handle it depends on timing:
- Within 5 days of your usual injection day: take the missed dose as soon as you remember, then return to your normal weekly schedule.
- More than 5 days after your usual day: skip the missed dose entirely and resume your regular schedule the following week. Do not take two doses in quick succession.

Side effects: what to expect at 7.2mg
Patients who have tolerated Wegovy at 2.4mg generally find the 7.2mg dose manageable, particularly when the increase is made gradually. The side effect profile follows the same pattern as lower doses, predominantly gastrointestinal symptoms that are most noticeable during dose changes.
The most commonly reported effects at 7.2mg include nausea, vomiting, diarrhoea, constipation and abdominal discomfort. These tend to be mild to moderate in severity and typically ease once the body has adjusted to the new dose.
One additional effect noted specifically at the 7.2mg level in the STEP UP trial is dysaesthesia — a sensation of tingling or altered skin sensitivity. In the majority of cases this was mild and resolved on its own without any change to treatment.
